Change to prescription and sick note requests

From 1st May the Practice will no longer be taking medication requests over the telephone.

Patients can email the Practice [email protected] with their request. Post the prescription in the red box provided in downstairs reception at Armley Moor Health Centre or via your pharmacy.

You can also request online access to your prescribing records to request your prescription online. Only your current repeat prescription will be available online..

The Practice requires 48 hours notice to request your prescription but it may take longer for your Pharmacy to dispense. If you are ordering via your Pharmacy please request at least 7 days before you run out of your medication.

Following the lifting of covid restrictions the Practice will no longer be posting sick notes. You can call the surgery to request a sick note but this will either be emailed, sent as an attachment to your mobile phone or you can pick up in person from the surgery reception desk. If you are nominating a friend or relative to collect for you, could you please ask them to bring some form of ID.
