Social Prescribing Support

We would like to tell you about a service we have available at the surgery that you may be able to benefit from.
We have a team of Social Prescribers here at the surgery who can support you with offer you an appointment for up to an hour and a half. They will talk to you about any social factors that are contributing to your health and wellbeing and tell you about lots of different services, projects and groups who may be able to help you. Social Factors that affect our health include things like money management, how we spend our time, our mental health, our friend and family networks and our physical health and long-term condition management. If you are interested to find out more contact reception who will book you in for an initial call with a member of the Social Prescribing Team.
